Referrals

All palliative care services use the Service Coordination Tool Templates (SCTT) and Guidelines as developed by the Victorian Government.

People may self refer to PHS but are required to complete the following five forms:

Confidential Referral Cover Sheet
Consumer Information
Summary of Referral Information
Consumer Consent to Share Information
Palliative Care Supplement

Once you have downloaded and completed the required five forms as listed above, please fax them to PHS on 9770 2798.
