Triangle of care
PHS is a key player in the Peninsula ‘triangle of care’ model which comprises acute hospitals, inpatient palliative care unit and home-based palliative care services.

Partners
Well established relationships exist with the Royal District Nursing Service (RDNS), Peninsula Health, Peninsula GP Network, Residential Aged Care Facilities, Monash University (through the Vivian Bullwinkel Chair in Palliative Care Nursing) and other community services enabling clients to have home care plus access to acute, respite and inpatient palliative care services and other community resources as required.
