KEY CONTACT

PHS recognises that during a time of ill health, often a number of people and services may be involved in providing a range of support. As this may sometimes become overwhelming, the Key Contact role has been developed to help navigate through the labyrinth of support options.

What is the role of your Key Contact?

Your Key Contact person at PHS:

  • Maintains contact with you on a regular basis to ensure your needs are being met
  • Is the best person for you to contact at PHS during office hours as they will be aware of you and your situation
  • Will help link you to other available PHS services (eg: groups, volunteers)
  • Communicates your agreed upon plan of care to other relevant professionals, both internal and external to PHS, as stated on your consent form
  • Will assist you with any queries or difficulties in a non-judgmental and confidential manner
