| The Peninsula Hospice Service (PHS) provides care
at home for people living with a progressive life threatening illness.
Services include nursing and medical
care, counselling, pastoral
care, art and music
therapy, social work and
welfare support, bereavement
and patient care volunteer support.
Support is offered to families and carers in addition to patients.
Over recent years, evolution of a ‘triangle of care’
has occurred, incorporating acute hospital, inpatient hospice units
and home-based palliative care services. PHS is a vital player in
this best practice model, providing care at home to the residents
of the Mornington Peninsula right up to Mordialloc. PHS has successfully
established positive relationships with the Royal District Nursing
Service (RDNS) and Peninsula Health enabling patients to have access
to acute, respite and inpatient palliative care services and other
community resources as required. Patients and their families are
able to access relevant and seamless service delivery from professionals
and volunteers within their home or hospital environment.
Continual communication between PHS, external personnel, patients and
carers is a high priority and enables the patient and their carers to feel
totally supported within the environment of their choice.
All services offered by PHS are free, apart from the RDNS fee which
is calculated according to the patients’ income. No person
will be refused nursing care if unable to pay the RDNS fee.
